Abstract: In dose escalation studies, the potential ethical costs of administering high doses must be weighted against the added utility from gaining safety information about high (and potentially effective doses). This is the rationale for starting with low doses while the confidence in the safety of the drug is low, and escalating to higher doses as confidence grow. A decision theoretical framework is proposed.
